(APPLICATION FOR SERVICE MEMBERSHIP)
THE UNDERSIGNED hereby applies for Service Membership in the Old Spanish Trail Association in accordance with the conditions, benefits and dues outlined hereon and agrees to pay for such membership as follows:
(	)	$20.00	cash	or	check	herewith.	(	)	$22.00	on	terms of $7.00 herewith; $7.50 in thirty days and $7.50 in sixty days.
Membership to run to December 31, 1928.

It is understood that should such member fail to maintain the high standard of service upon which membership in and official approval of the Association are based, the Association has the right to revoke the membership and return the unearned portion of dues, if any, paid the Association ami the former member agrees to return upon demand any and all property of the Association in his possession.
